| NASD Athletic Information Spring 2011-12 Sports Physicals will be held on Saturday, February 11, 2012 and in the Training Room at the Senior High School. Athletes must have the proper information signed by their parents or guardians prior to being seen by the doctor. To be able to participate in Interscholastic Athletics all paperwork must be returned and filled out correctly. The physicals must be dated after June 1st each year and are good for one calendar year. For School Physicals: Please have a check for $25.00 payable to "St. Luke's Physician Group, Inc." Athletes may also receive physicals from other certified physicians of their choosing and return the paperwork to the Athletic Office by Friday, February 24, 2012. 2011-12 Sign Up Procedure Sign-ups will take place in the High School Athletic Office for grades 9-12 and in the Middle School Main Office for grades 7 and 8. Students will sign the master lists and pick up packets containing directions, physical forms and extra-curricular code of conduct.
